Transitional outpatient care, or partial hospitalization, provides structure and support
This transitional outpatient care program is designed for patients stepping down from a higher level of care or who are suffering from significant emotional or mental distress that makes it difficult for them to manage their life in a work, school, or home setting. Partial hospitalization offers the benefits of inpatient care without the overnight stay. This allows patients to stay connected with loved ones and their community, while providing the safety and support they need to manage their illness.
This holistic program features:
- A structured, safe therapeutic environment, Monday – Friday from 9 am to 3 pm
- A supportive group format that reduces feelings of isolation and hopelessness
- Extensive group therapy that focuses on crisis management, illness education, emotion regulation and communication skills
- Recreation and expressive arts activities
- Mindfulness skills training
- Multidisciplinary treatment team and thorough discharge planning
- Lunch and snacks
- A variable length of stay from 2 – 14 days
