Following a visit to your SCL provider, your Discharge Summary should be available via your MyChart Account.
To find and download your Discharge Summary, please follow these steps:
- Log into your MyChart account.
- At the top of the page, click on the Appointments Tab.
- In the 2nd column there is a link entitled, “Admission Summaries,”
- Once you click on this, you should see your history of hospital admissions.
- If you double-click on the Discharge Summary you want, it should be printable.
However, if you do not see your Discharge Summary, you may need to contact our Health Information Management Department. To quickly get your medical information, please follow the below steps.
- We require this form be filled out by the patient before any Personal Health Information is released:

- Scroll down until you reach Step 3: Submit Your Request Form. This section will give you specific details on how to submit your form by Fax, Postal Service, or Email to the correct hospital. This webpage will also provide a phone number so you can call with any additional questions.

Obtaining Your Medical Record
If you need your medical records, we will be happy to send them to you. We work to make the process as convenient and timely as possible, while also taking the necessary steps to make sure we protect your privacy and identity.
Access to medical records is available to patients and legal guardians in the following ways:
MyChart
Get secure online record access to most records through MyChart.
- Don't have an account?
Create an online patient account for a safe and simple way to access information from your electronic health record (EHR). Once you have an account, you can also download or share records with others through MyChart. Some records may not be available online, but you can request copies of those additional records through your MyChart account at any time. MyChart is also available both via mobile app (iPhone, Android) and web browser. Records can be provided in an electronic format, if preferred.
- Granting friends and family access to your MyChart medical record.
Individuals 18 years of age or older may grant friends and family proxy access to their MyChart account at any time by logging in and granting access directly through the portal. Parents or legal guardians of minor children up to age 11 may request proxy access to their child’s MyChart account by completing the MyChart Proxy Form.
Parents or legal guardians of minor children from ages 12 - 17 may request proxy access to their child’s MyChart account by completing the MyChart Proxy Form, or the child may create their own MyChart account and grant access directly to their parent.
All medical information that is made available in MyChart will also be made available to your proxy. You control who can have proxy access to your MyChart account by granting and revoking access at any time from within your account.
Request Copies of Your Medical Records
If you've been a patient in one of our hospitals or SCL Health Medical Group physician practices, you may request access to your medical record by downloading a copy of the Patient Request to Access Medical Records Form (also available in Spanish), complete it and mail, email or fax it to our Centralized Release of Information Department or your local Health Information Management department to request your medical record.
Copies of medical records may be released to a third party (someone other than the patient) upon receipt of written authorization of the patient or guardian. To authorize the release of your records, complete the form below, then you may submit the form by email, fax or mail. Charges may apply.

Fees
Charges may apply for medical record copies or CD/DVD radiology images released.
Medical record printed pages:
- Patient: no charge.
- Non-patient: additional fees apply as allowed by the state. Please note there are NO charges for copies sent directly to another healthcare provider for continuing care.
- Another healthcare provider: no charge
Radiology images:
- First copy of radiology images is free to the patient. Please note there are NO charges for copies sent directly to another healthcare provider for continuing care. Please note that CD/DVDs can contain more than one exam. Copies of CD/DVDs incur a charge of $10 for third party requestors.
- Radiology images may also be emailed to the patient free of charge.
Birth Certificate Information
While we can provide medical information and records, we cannot provide copies of birth certificates. For a copy of a birth certificate, please contact Montana Public Health and Human Services at 406-444-2685, or Colorado Department of Human Services at 303-692-2200.
