We are a team of physicians and advanced practice professionals that focus on the aging-in-place population to provide acute and chronic care. This focus is specifically for:

  • Long term residents of nursing homes
  • Skilled nursing- acute stays within nursing facilities for oversight of medical management and rehabilitation services
  • Assisted and independent living facility patients

Skilled nursing and long term care services include:

  • Chronic care management
  • Collaboration with specialty services
  • Durable medical equipment referrals/ orders
  • Diagnostic testing – mobile lab and X-ray

Chronic Care Management

If you have Medicare or both Medicare and Medicaid, and have two or more chronic conditions, and live in either assisted living or independent living facilities, Medicare is offering Chronic Care Management services to help you manage your health and spend more time doing the things you enjoy.

  • A dedicated team of healthcare professionals who helps patients plan for better health. Services include monthly check-ins and ready access to the care team, including management of care transitions, referrals and follow-ups.
  • Comprehensive care plan. The plan will help support disease control and health management goals, including physical, mental, cognitive, psychosocial, functional, and environmental factors.
  • Annual Medicare wellness exam
  • Advance care planning

What is Chronic Care Management?

Providing care management and coordination of services to patients with two or more chronic conditions. Below examples include but are not limited to:

  • Alzheimer’s disease & related dementia
  • Arthritis (osteoarthritis & rheumatoid)
  • Asthma
  • Atrial fibrillation
  • Cancer
  • Cardiovascular disease
  • Chronic obstructive pulmonary disease
