The Vision Care Plan operates much like a PPO — see any vision care provider, but you will receive the greatest benefits if you choose one of the more than 65,000 nationwide EyeMed Insight network providers.
Click on "Begin Your Search" below and select "Insight" from the Network drop-down to find a variety of providers and where they are located. Even if a provider is displayed, we recommend you call ahead to confirm the provider accepts the Intermountain Healthcare EyeMed plan.
