Comprehensive Care for Joint Replacement

SCL Health is participating in a Medicare initiative called the Comprehensive Care for Joint Replacement (CJR) model.

CJR “bundles” the payment of all the services that are provided to patients during the hospitalization for surgery and 90 days after discharge. This requires all doctors, hospitals, and post-acute care providers to work closely together to provide high-quality care to patients undergoing total knee and hip replacement.

To provide more coordinated services, we have entered into Collaborator Agreements with orthopedic surgeons who perform total knee and hip replacement surgeries at our hospital.  See the Comprehensive Care for Joint Replacement (CJR) Collaborator Selection Criteria policy.

Additionally, we will provide Orthopedic Navigators, who will work with patients prior to surgery and will follow the patients for 90 days after discharge to make sure recovery is going well and to coordinate any follow-up care.
