What to Expect – Home Health

Each person we care for is unique and important. No matter what your path to healing looks like, we’ll be by your side every step of the way.

Getting a referral

Home health services require a referral from your doctor, who will determine if you are eligible for home health. It is important that we work with your doctor at the start and throughout treatment in order to provide the best care possible. We partner with primary care doctors, surgeons, nursing homes and other medical providers.

Your doctor can fax a referral to 303-388-1003, and we will contact you to answer any questions you may have.

Transition to home

If you’re in the hospital, a home health care coordinator will visit you and assist the discharge team with your transition home. They’ll also help you arrange a follow-up appointment and answer any of your questions.

If you’re not in the hospital, one of our home health staff members will call to schedule the first appointment in your home.

Your first home health visit

Your first home health visit may take up to two hours. We’ll discuss medications, insurance information and, if applicable, discharge paperwork from your recent facility stay. You’re welcome to invite a loved one or caregiver to attend this first visit.

Care plan

We’ll work with you and your loved ones, doctors, nurses, therapists and other caregivers to create a care plan that meets your individual needs. This includes making sure you understand your condition, treatments, medications and anything else you might have questions about. We’ll communicate with your doctor and other caregivers to keep everyone up-to-date on your progress.

On-call services

Our office hours are 8 a.m. to 5 p.m., Monday through Friday. We also have nurses on call 24/7 to help with after-hours issues.

Payment

Medical home health care is covered 100 percent by Medicare; however, if you have a Medicare Replacement Plan there may be additional expenses. We accept commercial insurance plans and usually require pre-authorization to determine coverage and benefits. If you have any questions, give us a call at 303-388-7000.
