When you leave our hospital, we don’t stop caring about your health. If you are transitioning to a long-term care facility or rehab, we provide an extra layer of support so you can live as comfortably and fully as possible.
Who we are
We are a team of doctors and advanced practice professionals focusing on transitional care and geriatric issues. We manage both acute and chronic needs in skilled nursing and long-term care facilities. We work closely with primary care providers, hospice, and other ancillary services to achieve optimal quality care. Our long-term care team is available Monday to Friday, 8 a.m.-5 p.m. We have access to physicians and nurse practitioners 24 hours a day, 365 days a year.
Philosophy of care
Our goal is to meet the needs of the patient throughout all stages of health when the patient leaves the hospital for short term rehabilitative care in a skilled nursing facility before transitioning back home. We also care for patients in long-term care facilities, bringing quality, professional health care to our patients.
