Many patients prefer to be cared for in the comfort of their own space, whether that is your private residence, a nursing home, or another setting. If you desire to be at home, surrounded by family and friends, we will do what we can to make that happen.

Our nurses, doctors, volunteers and others partners with you to provide personalized care. We are dedicated to coming to you and providing your family with resources to help everyone feel as comfortable as possible. Your family and caregivers will have access to round-the-clock expertise from our nursing staff to ensure that pain relief and other needs are properly managed.

Every family’s needs are different. If you or your loved one need inpatient hospice care, Collier Hospice Center is Colorado’s first facility designed specifically for end-of-life care. Set amid pleasant landscaped gardens, Collier Hospice Center is located on the Lutheran Medical Center campus. The 30,000 square foot center is Colorado's first facility designed and built specifically for end-of-life care and is licensed as an acute hospice care facility.

Collier provides a place for you or your loved one to spend the last days of life in peace. Collier Hospice Center is accredited by The Joint Commission on Accreditation of Healthcare Organizations. This means we follow a set of strict standards for measuring healthcare quality as we provide your care. At Collier, you also have access to our full-service hospital and all its services in case you need them.

Patients are admitted to Collier Hospice Center when they require 24-hour intensive care interventions to alleviate acute uncontrolled pain and symptoms of their end-of-life disease that are causing distress and discomfort. Care interventions can include medications, care needs assessment, safety education, and emotional support. CHC staff members also assist with arranging for the patient's continued care in another location. Once patients no longer require 24-hour skilled intervention, they may leave CHC and return to their previous living situation or to an alternative living site.
