Advance care planning

If you become unable to make decisions about your care, you can empower medical staff and family members to make decisions for you. Make your healthcare decisions known ahead of time by completing advance care planning documents (also known as advance directives), such as a power of attorney, living will and organ donor card.

Medical records

If you need copies of your health records from the location where you received care, we’ll be happy to send them to you. Follow the instructions completely and accurately so we can verify and protect your identity when sending you this confidential information.
